Besides Alfred got to the cafe because the necklace tracker led him there in the first place since Selina wears it in that final scene. It was Bruce's way of getting Afred to come there.

They didn't show Batman at the same time as the timer because the timer was on the bomb that was hanging 20 feet below the Bat. Unless they did some sort of split-screen thing, there was no way to show the timer and Batman. They showed Batman. Then they showed the timer.
Alfred dreamed of Selina because, in the past 8 years, Batman has only touched two women--one was Talia Al Ghul (aka the woman who probably fatally stabbed him if he somehow survived the explosion) and the other was Catwoman. He kissed Catwoman before he flew to his doom and I guess Gordon might have mentioned it to him. He was at Bruce Wayne's funeral with Blake and Fox. Some communication had to be made between the three.
There was a tracker in the necklace but Alfred no longer had access to the Bat-Cave and its technology. You asked why Alfred would imagine a theif? The better question is would Bruce Wayne give the necklace of his murdered mother to a woman who cost him his entire fortune, led him to Bane so that he could get his back broken, killed people, and kicked his cane out from under him when he was an invalid. Maybe Bane punched him in the head harder than we realized.
Face it. Batman is dead.

I'm gonna with the real-life logic approach here since it's what I was thinking about for the 5 or so minutes when you think he exploded with the bomb:
DC Comics would never have let Nolan kill off Batman. So to me that rules out the dream theory.

Spoilers: DC let Superman die at the end of All Star Superman and they let they let Joker die at the end of Arkham City and Batman 1989. I think that as long as he's alive in the comics, the other media aren't canon so anything goes.
Having only seen Nolan's work in Insomnia and Memento (my favorite movie), I had no idea he was a twist ending guy. I never saw Inception. I thought he lived. Then, I thought he died. I've been going both ways on the topic ever since. Right now, I'm on the "He's dead" state of mind. If you think he lived, he lived. There's enough stuff sprinkled throughout the movie for it to have gone either way. It's what makes the movie awesome.
