Keep in mind, nobody goes through and designs the whole freaking game on their own anymore, it's all done as a puzzle and put together at the end.
Imagine trying to paint a puzzles picture while all the pieces are apart. then you put it together and hope the picture lines up to something that looks nice. that's basically what game development is.
Oh yeah, and every 5 - 20 pieces is painted by a completely different person.
