it depends on the quality of the doctor.  if they put the silicone in there underneath the gspot, it actually pushes out the flesh and helps make the nerves get closer to the surface of the skin.  At least that's as far as I could figure it.  it's not like they're replacing the nerves and tissue with silicone, it's just putting a bulge in there to push harder on the penis.  I also think it helps to make the vagina tighter...
