by that logic we should give Guild wras 2 the credit as that is an MMO which requires the ability to handle dozens of human controled avatars all that have a combination of player selected items out of a pool of hundreds if not thousands, with all the other aspects of an open world game.
And aparently the Witcher 2 is going to be much more open than the first with large inviroments and no loading exept for chapter changes aparently. And if this enterview is to be belived http://www.faceoffgames.com/section/1164240 the witcher 2 will be "Second thing is, we've got a fully open world right now, so you can explore all parts of locations from the very beginning, so many quests are presented to you at an early stage." so the game is at least partially an open world game.
