Well for movies it can be due to the production values.... I'd argue that LOTR probably had the best costumes, best FX, best many things due to the budget they had to do it and the people being actual fans so doing the job doubly well... But yes that is a special case.
In games, well you're correct, although the budget is still a factor, it is very less so, and there is no reasons why one game should have all the honors.
Sadly the way people do this is by increasing the categories so that the game of the year contender don't even compete with each other.
