The front wheel had fallen off his bike, he had gone over the handlebars and his face hit the ground. Unfortunately his head continued to move forward while his face remained somewhat fixed to the ground; the result being that his entire face, from the mid forehead to his chin was shifted downward about 5cm.  He suffered no fractures or significant head injury and was concious through the entire ordeal.  The major issue when he arrived in the Emergency Department was that he was having difficulty breathing.  It was initially assumed it was just the large quantity of blood that was poring into his mouth due to his facial injuries but on inspection it turned out he was choking on his own nose.
