Yep, it's a about how violent the environment as a whole is. GTA is rated extremely high because you CAN be overly violent even though the game itself doesn't require it...
ESRB sometimes frowns upon sex and explicit murder on TV/film as well, but only if they use filthy language while doing it mind you... 
It sure is a strange world, how about the children of the Dark Ages who watched public beheading and hangings among other things? How did they survive?!
