Every time you end up juggling Idiot Balls to cover a badly written plot point. Ie. The ending of Fallout 3, and every single time you end up giving the keys to armageddon for the main villain because he captured your grandmothers sisters kittens or something. Aeris's infamous death scene in FF7 also counts as one, when compared to a similar scene in FF5.
