I have the same issue. Does it need to be completely filled out to register, or do you have to do it every time? I don't live in the US, so I can't fill it out completely.

Everytime i turn on my computer it ask me to fill it. I have filled it twice and since that i have just canceled it.

Weird. It never prompts me for any kind of input. Obviously one of us has a borked copy of the program.
