Because while MGS4: Guns of the Patriots would be on PS3, MGS4+O would be on Xbox 360.
You must understand, these are two completely different games.
Sure, in reality, it might just be a port with extra content.
However, in that same reality, Playstation 3 only owners cannot play MGS4+O. It is not for the Playstation 3. They can play MGS4: Guns of the Patriots, but not MGS4+O. You see, MGS4: Guns of the Patriots, and MGS4+O are two different games, that have at least some differentiation in content.
So, while you might just wave off the +O part, Microsoft and Kojima choose not to. In fact, I would imagine that the +O part could make the Xbox 360 "port" the definitive version of the MGS4 franchise, and the Playstation 3 edition of the MGS4 franchise, the inferior version of the game.
That is, if this is at all true.
I think it's too good to be true. If all this stuff was true, MS won E3.
