Let's try to guess what it is. I have three suggestions:
- Coding error (wrong field/table)
- Connection leak (out of connections)
- Invalid connection state
The last two aren't likely IMHO. If there's a connection pool on the other end, the behaviour is too consistent, but I won't rule them out.
