You could look at it from a few ways:
1) ESRB pressure to lower the rating for a bigger audience. Similar to the R-Rated effect in movies, and how many get hacked off to fit a PG-13 format
2) Media pressure not to add in controversial material such as Mass Effect's lesbian scene
3) Fear of racism in certain games. Resident Evil 5 may have had some backlash due to most zombies being black (which, AFAIK have changed to a more diverse background).
