It can most of the time, but not all the time. It does so by tracking the gravitational force. When you swing the remote around, it can't really tell which of the registered forces is the gravitational force and loses the sense of direction (which way is up). Also, when you twist it in the horizontal plane, there is a force associated with the rotation. If the sensors are fine enough, it should be able to keep track. :)
The problem of using that in a driving game is that this measurement might not be precise enough to "remember" which way it was pointing to the TV (which should be for a game to look straight), that is why it is done at an angle because it is easy to track the gravitational force as it's always there and much stronger then a slight centripetal force.
