I've beaten it without dying but I did use special weapons. Never tried to do it without special weapons.

EDIT: Someone mentioned StarTropics 1 and 2, beaten both of those w/o any cheats. No need for them really; they're not that hard of games IMO. The last parts of 1 are pretty tough I guess.
