They aren't characters per se, but Striders from HL2 were pretty intimidating. They has War of the Worlds written all over them, further reinforced by the firepower they had.

Mr. Bubbles from BioShock. I was so scared not to shoot one by accident before I was prepared with the area that I didn't fire a single round while they were around, even if there were 10 splicers as well.

Thadius from Metroid Prime. Maybe it was the fact he rose from the ground from a bunch of rocks or the fact that I never have once seen him in his entirety on one screen.

Baal from D2: Lord of Destruction. You saw that opening movie too, the one with the poor barbarian messenger.

Hellknights from DOOM3. Let's face it, almost everything in DOOM3 scared you shitless, but somethings were "OMG it jumped from the shadows" type of fear, the "I'm getting swarmed by demon babies" type of fear, and then there was the "I have the pistol and the shotgun against a Hellknight and the way behind me just closed" type of fear.

Counselors from HL2:ep2. If you've played through episode 2 you know the reasons, I don't want to spoil it for anyone else. Props to Valve for the helplessness they instill in the player.
