It's too hard for me to decide on just one for the PC but here it goes:
PC - Quake II - I spent so much time playing the multiplayer DM games, I think the map was called Edge. I'm pretty sure it single handedly made me the gamer I am right now.)
PC Honorable Mentions - All Blizzard and Valve Games (starting with Rock and Roll Racing and Blackthorne) - All of them changed gaming for the whole industry, PC or Console, in some way or another. All of them were the pinncles of gaming of their times and they are still better than any other PC or Console game that has come out so far. Only older id games can compare to their greatness.
Famicom - Contra - Before Quake II for me there was Contra. One of my proudest statements as a gamer is that I can beat all of Contra with just those initial 3 lives. Still can.
Famicom Honorable Mentions - Battle Tanks& SMB3 - (not sure the name was battle Tanks but yeah) The tank game I spent endless hours playing with my friend and screwing him over by freezing him when I shot him. And SMB3 was just the most expansive experience of gaming I had ever touched on the Famicom.
SNES - Link to the Past
Gamecube - Metroid Prime - Up until Galaxy and the overblown console titles like GTA IV and the Orange Box its 2nd place as best of the best was very well deserved. It's still #2 in my heart.
N64 - Ocarina of Time - Still #1!

You just managed to break just about every rule that the OP put down.
