1) impossible. No way for the Wii to tell the relative difference in positions between the Wiimote and Nunchuck (unfortunately)
2) like Wii Sports. I suppose one hand would work better though
3) could work. Don't know what would happen when you changed the angle of one and not the other. Seems like a more natural position than holding only the remote sideways (going from pic)
4) Guesture based probably similar to Godfather is possible. You wouldn't get great mapping due to being unable to tell position, but it would definitely be possible. Not great, though
