KOTOR. I'm a Star Wars fan, and it had an authentic feel about it despite the new cast and setting. FFVI and Chrono Trigger deserve honourable mentions also.

They're either pushing the game for a Teen-Rated, or they actually believe those themes would put the game AO-Rated (which is total BS, only once a non-porn game was rated AO, and that was because of heavy torture scenes, which Fallout games never had)
Bethesda specificially said that the game would be rated M for Mature and it would have the mature themes of past games.  This non-porn game you are talking is Indigo Prophecy, isn't it?

No. Pete Hines said they're removing child-killing, drug addiction and several other themes. That is what it looks like it's going to happen now. 

The game was The Punisher (it got AO rated, but they changed the torture scenes before release so that it got an M)
