Not only does this give out too much personal info but this info could easily be used for Voter Fraud. Someone could use this to show up at your polling place and vote in your name.

Lol did you just post your address? I'm registered to vote but that seems a little too far for me.  You might want to delete that top line.  Perhaps just change the title to are your registered to vote and skip any personal info posting?
