And regarding OP's question (well, OP is a savvy guy mentioning my name :P)...of course I would play such game. Everytime I get to create my own character, I try to make a male character as feminine as possible; although feminine guys are not transsexuals. I always found interesting that at the beginning of Dark Souls 2, you could get your character into a coffin that would change his/her sex. 

 

On a side-related note, I'd say transsexuality is underrepresented in gaming. I don't mean this as to blame game developers or something, far from it. I understand something as transsexuality must be treated in a tasteful way, as doing it for shocking value might be simply crude (this could work in games such as Hotline Miami, but that's not the point I'm trying to make); simply including transsexuals for the sake of doing so doesn't add anything at all. I wouldn't mind somethign like that either, but it wouldn't be meaningful.

*possible spoilers ahead*

Someone mentioned Nier back there, and while that specific character isn't transsexual - more like intersexual -, she struggles with her identity for most of the game in a very mature-ish way. Another great example, the most shining one, is Erica in Catherine, on which she's introduced to the characters in a very natural way, and hints (too obvious yet I was oblivious to those) are given to the player regarding her nature. Still, you don't get nothing obvious and specific, and the group of characters accept Erica in their friend circle without any kind of problem; in fact, one of the friends - Toby - falls in love with her (though he's the one oblivious to Erica's nature as well, which comically everyone else try to explain it to him but he doesn't understand until he has sex with her). This, coupled by the fact that Erica is the only female character in the game that experiences the nightmares leaves what's probably one of the most interesting side-characters in a videogame I've ever met; which actually makes me wondering if Atlus is going to pull something like that again in Persona 5 (I've heard Persona 4 has a similar character, but on the other way around).

Another game that also toyed with this concept was Fable 2's in its last side-quest mission on the Fairfax castle. You get the chance to alter your sex by drinking the Transmogrification Potion, and that change is permanent (probably the last important choice you have to make in the whole game). If you do so, and change your sex, people would remark on the streets things as "Didn't you used to be a man/woman?". I don't know if this was played for the laughs, but all things considered (and the fact that you can't revert to your previous self) it was a strange, nice touch nonetheless.

I might be dwellving too much into this, but these kind of unexplored themes are interesting; at least to me. Few devs have tried to break the mold outside the typical play for laughs - I'm thinking right now of Dragon Age, which, all things considereded regarding Bioware, the few instance of a transgender character was an elf prostitute put mostly for shock value (and I think a dwarf transgender too!) -. Nothing wrong with this, as I don't want developers including these kind of characters for some SJW bullshit pressure. I just kind of wish...you know, a few more games that explore this. That, or when I finish my developing course I'll do it myself!
