In my opinion, nothing comes even close to Mitsuda's score for Xenogears. Even his own great work on other games (Chrono Trigger, Chrono Cross, etc) can't match the atmosphere his music helps create.

Modern Warfare 2. Crap game, great music. Probably because they hired Hans Zimmer.

GTA: San Andreas

They have Killing in the Name by Rage Against the Machine and is therefore completely awesome.

Actual game music though:

Final Fantasy VI, VII and X had fitting music

Mass Effect 1 & 2 (some of the tracks kinda remind me of Pendulum)

Shadow of the Colossus

Elder Scrolls III & IV (the music just works for the settings)

Jeremy Soule is a genius.

Regarding GTA, I like the loading music and intro music for GTA: SA. I even had it as a ringtone.

Rockstar: Announce Bully 2 already and make gamers proud!

Kojima: Come out with Project S already!

I don't think any game can match the complete soundtrack of World of Warcraft (including all the expansions).

Theme-song wise i'd say one of: Monkey Island, Zelda, Secret of Mana

Easy, Streets of Rage 1 and 2.

I loved Assassins Creed 2 themes. They were brilliant:

These two are the best. Some other honourable mentions are Metal gear solid 4 old snake theme, and Still alive from Mirror's Edge.

EDIT: Holy Crap can't believe I forgot this:

Take a look at my photos on flickr

Chrono Trigger all the way, Terranigma, Legend of Zelda a Link to the Past and The World Ends With You. Even after years of beating these games their music still comes to my mind every now and then.

Chrono Trigger

Chrono Cross

Xenogears/Xenosaga Episode 1 and Episode 3/Xenoblade

Grandia 2

Final Fantasy 8 (listening to it alot lately, and it reminded me how amazing the music was)
