Well, Under the Knife 2 was a major fucker. X5 was a real problem. Every attack did 25 vital damage, a syringe of stabilizer recovered 12, you had to inject two, that took 3 seconds, you had to stitch the wounds which took two seconds, and you were allowed to attack for a second.
To give an example, say you were in an RPG. You had 9900 health. But every attack this boss did took away 2500. Healing was worth 1200 points. You could cast it twice. It took three seconds. But now, there were evil leeches sucking your blood. These took away 100 HP every second for each there was. Well, you had to destroy them with three seperate rapid attacks which took two seconds. If there were three on you, 600 HP gone. And guess what? You have six seconds each turn. This leaves one second for attacking. Rinse and repeat. But those leeches make you lose HP that you can't recover! When you attacked it for five whole seconds and going down to 6000 HP, it explodes. But now there is its heart. It has six hearts. But guess what? There are magic little bugs going around the heart at like 40 miles per hour. There are about 30, and four of them home in on you. You have to avoid them and attack the heart. Now, you're allowed to heal, but watch out, you have eight seconds to attack the seemingly invulnerable heart. And if you touch the bugs, 2500 HP gone!
