Yes, it used readily available images on the internet. This however uses a bunch of images of people using a wii "pedal controller", as well as some Miis that don't look like Nintendo's Miis. All of that had to be created, rather than cut and pasted.
Far far more effort went into this image.
I think it's real. We'll see.

Or even an exercise bike.
Showing the bicycle thing at E3 would have been pointless, it couldn't have competed with the vitality sensor.
The bike thing would be huge, if they bring it out at a low price (compared to an excercise bike), and with software for keeping track of your excercise and some games/training routines
