Even though there's that game that came out in Japan that everyone claims to be the PS2's swan song, I argue that the PS2 is still healthy in Japan, in comparison to US/EU. So for those 2 regions I say GOW II was it's swan song. Absolutely gorgeous graphics, high production, great gameplay, awesome and simple story.

Super Famicom - SNES. I'm assuming this guy is Japanese both because of that and because he called Megaman "Rockman".
How does one define "last breath"? Last game made by the first party, the last month or two or is it a longer period of time? Or maybe just after the next console is made?
I'd have to say Yoshi's Island for the SNES. I don't think it even sold that well especially compared to other Mario games despite being a great game.
I'll come up with something better eventually...
OutRun 2006: Coast 2 Coast was a pretty good last hurrah for the XBox even though I could hardly find it anywhere (ended up buying a used copy at Block Buster) and MK: Arma wasn't too bad. Granted the XBox still had two years left (and maybe more now?) thanks to Madden 07,08 and maybe 09? If you can count those :P
People also forget Double Agent for XBox had the Chaos Theory team behind it, where as the 360 version had Team Shanghai. The result? A better game and more multiplayer options.
PS2 I would have to say Persona 4 but maybe Devil Summoner 2 will change that thought. Besides even with the price cut to $99 they still aren't flying off the shelves and looking at Gamestop's list there isn't much to look forward to. I would argue the PS2 will die sometime next year but maybe Atlus or NIS will prove us wrong.
Panzer Dragoon Saga was the best way to go out so far for any system.
GBA went out with FFVI before It's death in the US I believe.
I'm probably wrong but I could have sworn the last titles on the NES involved Mega Man 7 and Startropics 2.
It's just that simple.
