$22.99 in Canada, so $15 to $20 seems about right in the US. (according to Amazon http://www.amazon.ca/Rockstar-Games-39550-GTA-IV/dp/B001QD5JAQ).
It's $20, but some retail outlets are selling the codes for said price, but are including extras (like gold membership) for free.

In U.S. its 1600 MS points or $20.

I don't think any PS3 GTA IV DLC will happen any time soon//unless Sony forks up some money//360 DLC was bought by Microsoft.
