Final Fantasy 4 (US) or Final Fantasy 64. :(
I know it didn't even make it into alpha development (or post pre-production, whatever, I'm not a developer) but it still was never released.
I would cite regulation, but I know you will simply ignore it.

SF2 was Lylat Wars/Star Fox 64 on the N64.

No it was not. Lylat Wars borrowed some ideas from Star Fox 2. The original game was even presented at E3 back then.
After updating the Star Fox games yesterday, Star Fox 2 comes to my mind as well.
The missing/cancelled Xenosaga games, even if a good portion of their story found its way into Episode III.
Runners up:
Eyedentify (the reason i bought my PS3)
Final Fantasy 64 :(
Lunar 3 (was mentioned in the booklet of Lunar 2: Eternal Blue for PlayStation)
Sonic Extreme (what i have seen on Youtube was promising in my eyes)
and Sin & Punishment 2... no wait. :p
Tempus fugit Nintendo manet.
