Elite Beat Agents for me, when the agents suddenly turn into hot women and everything starts going at light speed.
Other than that definitely Super Ghouls and Ghosts. I spent hundreds of hours trying to beat that game, yet never managed to. Hate the game with a passion even today.

Everybody says that the original Battletoads is the hardest but I never played it too much. I did played Battletoads in Battlemaniacs and I always thought it was impossible to finish. Played Battletoads vs Double Dragon and couldn't get far either. Are all Battletoads game made with this punishing difficulty?

Ghost ´n´Goblins on NES/C64. I remember playing that game as a kid and get so frustrated that I almost threw the controler in the wall, sometimes that game could make you so angry that you almost cried. For those who played Super Ghouls´n Ghosts and this, which one is hardest?
I would say Oddworld: Abe's Oddysee proved some difficulty to me. I was rather pleased with myself when I completed it.
The early Tomb Raider games are also generally quite challenging.
Alot of games have their difficult moments, those ones stick out in my mind. Maybe I just haven't played too many overly challenging games.
The Brain Training Stroop Test is too hard (to get the target time).
I had to answer the correct colours into the microphones correctly 50 times in 50 seconds or less! The game failed to recognise my normal voice so many times and I'm forced to fake my voice to play it properly!

Oh no doubt here, Touhou is fantastic, the music alone makes me want to keep playing. The game IS about memorizing pattern too, but Ikaruga's was just a lot more taxing on my brain when compare to Touhou, I too have been playing shmup for a very long time, too long perhaps because Ikaruga's black & white patterns completely mess with my sense.

Proof that no matter how hard you think a game, there will be someone out there who can do it with even more difficulties imposed. 
All 5 of those vids are a single guy playing both ships at once. The last one he has also filmed his hands to show you what he does. There are some other impressive videos on that site too, so be sure to look around.
