So, I guess I'll be half a monkey and say Ninja Gaiden Sigma(since it's part 1 and all.)
Kind of a odd question. A lot of games can be very hard when you ratchet up the difficulty setting.
Two off the top of my head are Halo 3 Legendary with all skulls. Bungie revealed only 2 people have done this while connected to XBL.
Ninja Gaiden Sigma on highest difficulty. There was only one person at Tecmo who could beat the game on this mode when it was released.

For me it's been Halo on Legendary with skulls, but going for the Saint achievement on Dead Rising has also been tough.
Also, I don't think Ninja Gaiden Sigma should be in the discussion since it's a port of an original xbox game.
i haven't played that far into it yet but every level in PixelJunk Monsters complete and saving every villager
