The pause that occurs when someone gets hit or dies. It's only an issue on multi-player, but still very annoying. Of course, the more players you have, the more likely it is to happen.
I was gonna debate your point, but you're pretty much right. I like how Green Stars add an element of difficulty scaling to the games, but the back-tracking to stars you missed is annoying every time. Bravo.
