I know its possible to connect the headset via USB/jack to the PC. 

What I need to know is if its possible to connect it to the PC via USB and just plug it into the PS4 controller whenever u need to use it on the PS4 without having to get up and disconnect it from the PC.

I think it's not possible. To make this headset works will need to plug the bluetooh controller through the usb port in one device only (pc, ps3 or ps4). When you change the device, you have to carry on the bluetooh controller and this will make the headset disconnect in the original device.
