He seems to do okay if he's calm and prepared, but if he gets flustered... ouch. His appearance at the 2011 VGAs was painful to watch. I felt so bad for him. 
Let's just say he's no Tak Fujii.

Many foreign 'stars' use English translators even when they speak English very well.
Just look at Ichiro.
So it's certainly a possibility that Kojima can speak English.

Thats my point though, he's probably been practicing for the day to shock us across these many years, including those which he used a translator, but he might have been better than he was before
