Most likely one of the Pokémon games. When I was younger the only thing I really played on my GBA/DS was Pokémon, so I played it a lot. I don't do so as much now (I play more games, duh) so they don't have as many replays.

I put up either Sapphire or Pearl as my most replayed game/s.

Aladdin on the SNES I think. Only because the game is really, really short.
Out of the games with a more "normal" length, hum... I think it's Shadow of the Colossus. Around 2 or 3 times. Though I've unlocked everything in DBZ Budokai 3 two-three times too.
