Does anyone know if Ground Zeroes is included into the Phanton Pain, like the bunker mission from MGS 2 and the release in spring is just for people to get to play the game and get used to the open world gameplay, or do we have to buy it to completely play MGS V?

Nobody knows but the Ground Zeroes will be launched at least a year before Phanton Pain...

That is an unknown at this point. All we know is Ground Zeros is Digital only Next-Gen. It is more than the MGS2 bunker level.

I assume full buy + MGSV
