Can't remember, but it's happened more than once. I have some pretty bad rage attacks when frustrated.

Same. I threw my PS3 controller at the wall while playing Rayman Legends once.

You're still lucky. When I get frustrated my chest pain becomes worse which fuels my aggresiveness. :(

That's why I try to avoid games that are too hard even though I liked them before I suffered from an anxiety disorder.
