Any reason why you don't want to stick with the Phenom 2 x6 1090T?
Those things can overclock to 4ghz relatively easily, I had one when they first launched a few years back and it's more than enough to handle most games out today.
Just get a decent cooler and you're set. :)
As for the Radeon 6950, is it a 2Gb reference card? You might be able to unlock it into a Radeon 6970. The 6950/6970 is already equivalent in games to the 7850/7870, so it's not like they're horrible yet.
If you are dead set on switching to Intel the best price/performance would be:
* Core i5 3570K. - Overclock it. :)
* ASRock Z77 Extreme4
* Coolermaster Hyper 212+
If you are having only 1 GPU, don't bother upgrading that PSU, save your money and keep it for another 3-5 years when you upgrade next.
If you can't stretch the budget to a Radeon 7950, then stick with the 6950, otherwise you would be spending money on a side grade. (Or buy a second Radeon 6950 second hand for crossfire)
Alternatively you could sell the Radeon 6950 and the Phenom 2 x6 1090T, that should give you an extra $300 - $400 USD to add to the overhaul.
As for Drives, grab yourself an SSD, Mechanical hard drives should not be allowed as primary storage, then use your old drive as a Steam drive or storage or something, I have a 2TB Western Digital black as my Steam drive and it's almost full, another couple of 2Tb Western Digital greens for storage, Windows on the 64Gb SSD. :)
And if you're still running out of space, buy a 2tb external hard drive and drop all your files onto it. :)
