Methink it was probably playing Demon's Souls. Oh, and a few months ago, it was the second time I lost my save of the Witcher 2. Waited three weeks before restrting in hard mode.

COD makes me rage all the time, not sure why. I rage but still have 'bout 1.5+ KDR, but nonetheless it makes me rage so bad, I once hit a controller on the desk and the battery pack flew off and hit me in the face, on top of that the corner of the controller busted open with half of the controllers innards coming out. I then proceeded to scoop everything in, reinsert the pack and go back to raging again.

Dark Souls too, gargoyles almost made me rage quit after I died around 15 times consecutively, then the lower swamp with that op poison, then got cursed by a giant rat. What made the game even worse is the fact that I lost the 25+ hour save when I transferred to my new Xbox, so I am actually just not picking it up anymore to replay that nightmare.

Hot Shots Golf (one for the PS2) was playing with my friends for money, on the last hole i chiped it from 30 yards, ball landed and was rolling perfectly for the cup when at the last secondthe ball slightly rolled away from the cup and when into a bunker. The part that got me was the green was flat and the ball shouldn't have moved like it did. I threw my controller and my PS2 went with it. I think my PS2 went farther than my controller lol.

I have a few rage moments when I play WoW in a bg. Especially when it goes like this. I am a warrior and I find another warrior and we are 1v1 each other. We are neck in neck and then we both hit around 21%. Just then my connection lags for an unknown reason at that specific time. Within the second that I lag I am dead. Normally after something like that happens I rage a lot but eventually find that same warr 10 more times to kill him without even caring about anything else.
