What happened and what difficulty was it?
Super Mario Galaxy 2 - The Perfect Run. You get hit once? Dead. Check points? No. To this day I still haven't got all 242 stars - got 241. I just get enraged with that level so bad.
Also on Mario Kart Wii - not getting a 3* rating you feel you deserve - even after coming first in every race and not falling off the edge. I actually smacked the Wii remote over my head. Ouch.

That's the classic for, "I didn't know what to say, so I'll just blame it on VGC." post. 
I rage at the drop of a hat. Used to do it often on VGChartz, now its mostly in the mod-chat, where i once threatened to declare a fatwa against Rocksteady over the Predator sections of Arkham City, or cursed a blue streak over various parts of Ninja Gaiden 3. I made numerous colorful accusations against BioWare over Knights of the Old Republic, once threatened to beat Elite Four Lance's head to a fine pulp with a shovel over Pokemon Soul Silver, tore a hole in my game guide over Ghetsis' Hydreigon for Pokemon White, left a large dent in one of my globes over the Maintenance Drone in Metroid Prime 2, and put a dent in my door in my room in Japan over Chapter 13 Eirika side of Fire Emblem: Sacred Stones.
Monster Hunter: pissing me off since 2010.
Mario Kart rubber banding coupled with blue shell rape when you are jumping a gap. Happens every single time to me in 150cc. Then, when you finally get lucky and manage to win all 4 races of a cup, you still get only two stars.
Fuck. This. Game.
Ongoing bet with think-man: He wins if MH4 releases in any shape or form on PSV in 2013, I win if it doesn't.
Dead or Alive: Dimensions
Mission 20 in Tag Team mode. I already had 1 enemy down and the other one at 1% health. Guess what, from that point on ALL of my attacks were countered and I eventually lost. I threw my 3DS across the room, but since it is a Nintendo product it still works^^
