I have literally never combed my hair. Ever.

Every morning and after a workout. Sometimes before bed if I was out in the sun.
Rarely. Only when my hair is long enough for it to matter. I keep my hair very short most of the time.
After every shower
twice a day. In the morning in the shower and before I go to bed. Though if I eat something with heavy garlic or raw onions I'll brush after the meal.
I don't use foot powder, my feet don't generally sweat. I put on a fresh pair every morning. I will also change my socks if I exercise.
After every shower. This keeps the wax from building up and it's softest at that time as well due to the hot water.
I cut my nails once a week. I have only had a Manicure/Pedicure once, I was dragged by my gf there, but I would do it again. It was very relaxing and the massaging part felt good. Plus they had an action movie playing of the tv. I was litterally having two women wait on me hand and foot.
Shaving - this is my laziest department. I shave twice a week, though I really should do it every day since I have no lack of growth in that department.

You should twist the Q-tip this way the wax sticks to it. If you just push it in and out, then I could see how that would push the wax further in. If you do it right you shouldn't get an infection. (I've never had one)
Speaking of ear cleaning. The creapest way I have done was with a bamboo stick. My ex-gf was Japanese and that is the popular way to do it in Japan. She would do it for me, pretty romantic, though pretty scarry the first few times.

They scoop the wax out with the curved end then swab out the left overs with the cotton end.

It doesn't say "Extra Strength" it just says "Stays on for 48 Hours!" (It is Dove Men.)
