
With the rumors of a new console around I guess it'll be interesting to see what peripheral/consoles/accesories had fail terrible in the last 5 years. And yes fail terrible; this thread is no place for "Duh Console "X" failed" this time we're going to dig into the bottom of the barrel.
HD DVD Player (XBOX360) 316k units sold

Well back in the early days of this generation we had a war, a war between the Blu Ray format from Sony and the HD DVD from Toshiba. It was expected to be something like the VHS Vs Beta, something that'll be decided by the audience but instead it was decided in the desk of some executives. Anyway Microsoft announced the HD DVD Player in 2006 and was discontinued in February of 2008. Microsoft support about this peripheral was sketchy, none of the games used the format and there weren't any plans to adapt the system to it. Anyway it's a nice collectible, and for a time people get 5 free HD DVD movies with it, the last units were sold for just $49.99
Wii Speak (Wii)

Well for years Wii users claimed for a micrphone/headset to do the things that every console user do with their headset (scream at a screen and curse unknow people) so their wish came true in 2008; kinda. The Wii speak is a peripheral that instead of the traditional headset, uses a stationary micrphone that allows voice chat with a entire room, ¿convenient? maybe but the design turn down a lot of users, also te lack of support for the peripheral is quite evident; so far only 10 games are compatible with it and there isn't any signs of future games that'll use this peripheral.
The Eye of Judgment (PS3) 320k units sold

One of the biggest buzz during 2006 E3, The Eye of Judgment was announced has one of the biggest jumps in TCG games, the title was in development for more than 3 years before hitting the PSN and the stores in 2007. The result was a expensive experience that few peple actually enjoyed, sure there was a nice little community devoted to the game but most of the TCG fans were quite vocal about the game (and system) faults. So after 2 and a half years Sony decided to cut the online support for it in 2010, a PSP game was released later and had worse sales and support.
Tony Hawk skateboard .... thing (PS3, XBOX360, Wii)

It's hard to believe that the name Tony Hawk was associated with excelence in gaming, but it's true Tony Hawk Pro Skater 2 is still one of the highest games in metacritics, but today the name is associated with bland gameplay, terrible cutscenes and most important one of the worst peripherals of all time. In fact the skateboard only has one problem .... it doesn't work, so after a bunch of terrible reviews Tony Hawk himself came to defend his product in one of the most embarrasing moments in gaming history; still if you want to get this piece of plastic as a collectible you can still grab one .... for 20 bucks.
Wii Junk (Wii)

Yes the Wii broke a lot of records, took gaming to a much bigger audience and earned a place in gaming history, so some companies decided to cash into the badwagon of sucess and designed a lot of peripherals to "enhace" the experience. Mostly garbage, the quality and use of these "peripherals" is highly cuestionable, and there're so many that you can literaly make a "Top" list with them (in fact some sites already had those list).
The PSP Go

Oh yes this's the most controversial entry into the list. So can we consider this handheld a failure? If we talk about sales the answer is yes, so far the number of units sold has been a subject of debate in many forums but it's true that the handheld reach 3 digits sales in Japan, North America and Europe doesn't seen to support the system either. So far most of the speculation points that the PSP Go is still at its first shippment and even with offers of free games included the sales are quite poor. Some people argue that the PSP Go is a success because more than 200 new titles were added to the PSN and it was just a test to check the market for new handheld without physical format; well both of these things could be achieved without releasing a new handheld so anyway if the PSP Go was a success or not it'll depend of the point of view of every person .... either way you can always blame Marcus.
