Thats because we don't have an R18+ rating for games. The step below that is MA15+ which is what a majority of the games which get 17+ ratings in the US are given. We would have an R18+ by now, except the SA Attorney General is so out of touch with technology and "worries" that these games would get into the hands of chilren.
As has been mentioned, either import it, or what a majority of people who really want the game end up doing, pirate it.
