Look like Igor Lab most likely found the issue with the adapters.

The summary is the adapters is using a metal bridge that connect 4 wires too the 6 power pins. When you bend back and forth it is easy for one or 2 of the wires to get disconnected from the metal bridge and now you got all the power going through possibly 2 cables which is very bad. It wont always happen but if the QA not good on the soldering to metal bridge it will.

Some of the power cables PSU makers selling that go into there PSU wont have this issue because they using 6 wires to 6 pins. removing that point of weakness. There was no risk on the adapters that came with 3090/ti also they was 6 wires to 6 pins.

It was the decision to go with 4 wires to 6 pins that meant if the soldering job was not perfect to metal bridge there is a major point of failure.
