Since no one has said it so far, I'll mention Cheetahmen II. That game gives Big Rigs: Over the Road Racing a run for its money:

In addition to the Cheetahmen, I also always though Heroes of the Lance was among the worst games ever. And that's one I actually played on a real NES cartridge.
