Most likely they have a tool searching through PSN users, though I imagine if PSN users have a bunch of privacy settings on their accounts they aren't counted.

And yeah Players will always be higher than actual sales, especially the longer the game is out and the number of used copies increases. PS+ also for some of the older PS4 titles.

Fortnite is noticeably absent from the top played games, it is on the website but listed under "Fortnite: Save the World" at just 10m players, so my thinking is that for some reason only the Pay To Play singleplayer campaign is being counted, maybe it's an issue to do with the game using Epic Accounts? Hard to say, but doesn't seem accurate for this particular title. There's no way Apex Legends is beating it.
