Even when AMD was superior to nVidia in Price/Performance/Power Consumption, they still fell short of marketshare.
GPU designs also tend to be fairly capital intensive, something that AMD didn't really have much of an abundance of over the last few years.
Ryzen should help turn that around though.

Intel pretty much stagnated after Sandy Bridge, which Intel capitalized on in order to maximize their profits.

nVidia most certainly hasn't. - Maxwell, Pascal, Volta and Ampere are all pretty potent designs with a considerably efficiency edge over AMD's archaic GCN based architectures.

Intel has signaled that it is entering the GPU space again due in 2020.
