It's not like Trump isn't equally popular in the real world.

As far as I see it's that Big D doesn't even have a majority in his own country and the rest of the world thinks of him as a joke. I'd say real world approval of Donald is safely below 10%. USA is probably the only country where his approval is above 10%.
