I have many problems with anxiety and stress myself (in fact I even made a thread similar to this a few months ago!!), but I have a few recommendations that may help you (even if I don't exactly manage to keep up with them myself):
1. Exercise - This is my biggest failure in terms of being consistent, but remaining active really does help with your mental state.
2. Sleep - I know that this is an area that you lack thoroughly. However, while you may be used to it and have adapted to it, 3 hours of sleep long-term has negative consequences.
3. Hobby/Goal - You need something that you can work on and have a sense of achievement out of. For me, that's learning languages. However, this site also gives me goals to work towards. I have the PS Nation thread that I work on that gives me a daily goal through updating it, and I've also been trying to get to know more people here and help them get more used to the community, as a sense of building things up for them.
4. Alcohol - I guess you already know, but it sounds like a problem. I don't know if you'll be able to get yourself off of it (and in fact if you're a heavy enough of a drinker, it's dangerous to do so), but I hope that you can do something that'll make you not so dependent on it.
5. Friends - You need someone to talk to in times of need. I don't know if you have people you know in person that can do so, but I hope that you have at least 1 person you can trust. Also, as you may know, I'm always open to having a talk
