I remember talking to John Hardin about it when he was still at Atlus, he mentioned that they tried to push SEGA to localize Ishin, but was easier said than done due to the nature of the game. Unlike Yakuza where you can get away with a surface level of Japanese culture knowledge, Ishin required a vast amount of knowledge of the Japanese era, and I guess SEGA wouldn't know how to advertise it versus the main series. So if it's a third Ishin game it will probably share the same fate as the first two sadly, which sucks because Ishin is a great game.
