Could be number of things.

 

  • He could be chomping down the on mouth the piece too hard.
  • The mouthpiece may be too big/small and affecting air flow, causing strain.
  • The mouthpiece could be vibrating his teeth and aggravating nerves, rubber mouthpieces are good for that.
  • The reed may be too hard. 
  • It could be the sound of the instrument itself, he can try wearing ear plugs while playing.
  • Or it could be a medical issue, though if the headaches aren't severe, it's likely not.
