We use to let our cats free, so they live how they want and have a place to eat or sleep whenever they want to, this one loved a wild lifestyle, he would go away for days straight, meet other cats, and naturally get into fights.

One time he got back all scratched, some time later he got back with a huge whole in his had, already oozing with infection, we took him to the vet, he had some bugs lay eggs on the injure and he had to go over surgery, he lost one ear to it and the damage went down all the day internally to his mouth.
Now he was acting strange, his head might have had kinda messed up, he would pee and shit over our house, which we don't let any cat do, and his mouth apparently got permanently screwed as he got a gingivits and the vet told us he would have to have his teeth taken away so he would stop drolling and having pain, but we don't want to do that and also don't have the money to keep on spending on surgery with him, sadly, so he lives like that, we castrated him so he gets around home way more than before, but he is almost always in pain, drolling and all dirty/smelly because of the droll, which further prevents him from entering the house, on top of the before mentioned. He developed a really sad expression because of the pain and not being able to get home as often as he wants to (he developed also a vengeance feeling towards us and whenver we let him home he'll take a shit right in the middle of the room or sleep and droll over the beds, lol).

He really feel sad for him, but aside from some painkiller here and there we can't do much to help, meanwhile the other cat is home all the time and is really happy, lovable and playful, it's even sadder to see the contrast between them both now =/

That sounds terrible! My cats have always been indoors only, I've had family members who had their cats run over by a car, so we don't want that to happen them.
