Well, I just looked it up on Urban Dictionary, and here's what it says:

1. Have a cigarette (British) 

2. Kill a homosexual (American)

So I guess that it's a British saying?

Also, in reference to your other post, they are already taking it anyways while it's illegal, so at least if it's legal they can seek help without fear of punishment, or they can go to centers that can give them clean needles to prevent the spread of diseases.
